TBH, after buying those practice exams, I don't find the NBME explanation super helpful. Most of the time, they give you a 1-liner reason for why the answer is right and then repeat that 1-liner in the "concept to learn" portion at the end of the explanations. Got the above the honors cut off for most of them and usually didn't finish UW. UW for shelf exams imo is literally worth 100x more as a learning tool than anything else. In order of easiest to hardest IMO. Peds - if you have the pathognomonic findings down, this exam was easy. Psych - know the psych drugs (1st and 2nd line) OBGYN - small amount of material to know, so 1 or 2 points off from the median can make your percentile jump like crazy. Surgical Recall will get you the pimping questions, Pestana's will get you most of the surgical subspecialties (Burn, Peds, Onc, etc.), NMS Surgery Casebook remains the best in-depth review for the general surgery portion of the shelf. I find there are more tricks and distractions than Iām used to. 48% puts me at a low percentile, like 4-6th percentile. I need 10th (65% ish correct) on the shelf to pass. Definitely try to get through UW Pulm/CC/Cardiology if you haven't yet. There just aren't that many surgery questions and the surgery shelf is very medicine heavy. DeVirgilio's is case files on steroids for surgery. I never got anything out of case files but DeVirgilio got me a 90% or something on the surgery shelf.

Lots of people fail it because they think they will get exposed to enough just through clinical work, which just isn't possible. The shelf itself covers a ton of different subspecialties and also just core concepts of IM/medical management that are unlikely to come up in the OR where ... This isn't the case for all shelfs, though. IM shelf last year had Ī¼=78 and Ļ=7 which does correlate to 76th percentile, but surgery shelf Ī¼=74 and Ļ=8, which correlates to 83rd percentile. Your school should have sent you a score report with your class and nat'l avg and sd (if they haven't yet, they might soon). I think 85-90% (raw) of the exam is preppable, but the last 10-15% is random ass nonsense within crit care, neph, GI, or card medicine that is extremely hard to get right. Most of my friends who got north of 250 on Step 1 ended up with 85-89% on the surgery shelf, with really intense prep.
